A 79-year-old man with medical history of type 2 diabetes, hypertension, and hyperlipidemia presented with a 5-year history of an asymptomatic, reddish, pedunculated mass of the right caruncle. The nodular mass was 4.3 mm × 3.0 mm, well-demarcated, and highly vascular without ulceration or friability (A). Excisional biopsy demonstrated a cystadenomatous tumor with tall, bland epithelial cells with abundant eosinophilic cytoplasm with a granular appearance, consistent with oncocytoma (B). Ocular oncocytomas are rare tumors with an incidence of 0.3 per million/year. Caruncular oncocytomas do not recur after total resection, so this patient has an excellent prognosis.
